Mr. Perfect in 1994-1995

What was the deal with Mr. Perfect returning to the WWF in 1994 and then leaving? He was supposed to wrestle Lex Luger on the WM X Revenge Tour following Wrestlemania, but disappeared just as the storyline got going. Did the WWF book him to wrestle without confirming it with him before he came back? Did he not want to put Luger over?
How did they get him to come back as an announcer for Survivor Series 1995 after he left them high and dry in ’94?
Also, was he in talks with WCW to be the man under the mask at Halloween Havoc which turned out to be The Butcher?

The 94 return and no-show was because he thought he could wrestle again, but his back injury ended up being too bad to return and there was some story about needing foot surgery as well. Plus there seems to have some miscommunication, where Vince thought he was coming back and Hennig never really promised to return to the ring or something.
As for why he came back as an announcer, Vince just really liked Hennig, I guess, because he put up with a lot of bullshit and trouble from him and even after he died of a drug overdose shortly removed from WWE and tried to shove Brock Lesnar out of a freaking plane, he STILL got into the Hall of Fame with everyone speaking glowingly about him.
And yeah, the Butcher was intended to be Hennig, in one of the many many many failed attempts for WCW to sign him. In this case he was actually backstage at the time and scheduled to debut before it fell through.
